Details

There were significant changes made to the PENEURO CRF. PPMI 1.0’s CRF had more variables, that were more detailed. They were specific to both the exam category (e.g. motor, sensory), and a certain region of the body where the exam took place (e.g. right hand, left leg). In contrast, PPMI 2.0’s CRF is simplified, with fewer variables. These variables are specific to the exam category (e.g. motor, mental, sensory), but each category’s result is recorded in only one variable. Thus, granularity on body region has been removed. Both PPMI 1.0 and 2.0 have variables that capture any comments made by the physician.

Listed below are the changes that occured for each category of exam.

Coordination

In PPMI 1.0, the coordination category was included under the larger Motor System category. It split out coordination into 8 variables, four of which contained the exam result for the specified body region (COFNLRSP, COFNRRSP, COHSLRSP, COHSRRSP), and four variables that contained any comments for that region (COFNLCM, COFNRCM, COHSLCM, COHSRCM).

In PPMI 2.0, the coordination exam is captured by two variables, one of which contains the overall coordination score (CORDRSP) and one that contains any comments (CORDCM). Furthermore, the 2.0 Coordination section specifies “Other than PD”, while PPMI 1.0 does not make this distinction.

Cranial Nerves

In PPMI 1.0, the cranial nerves exam result was located in the PECN table, split out by specific groupings of cranial nerves. For each grouping, there was also a variable that captured any comments. In PPMI 2.0, the cranial nerves exam has been condensed down to two variables, one containing the overall exam result (CNRSP) and one containing any comments (CNCM).

PPMI 1.0’s cranial nerve section covered nerves I-XII, whereas 2.0’s cranial nerve section covers nerves II-XII.

Gait

In PPMI 2.0, two variables were added that capture information on a new exam category: Gait (other than PD). These variables capture both the overall exam result (GAITRSP) and any comments (GAITCM). This was not captured in the 1.0 version of the PENEURO CRF.

Mental Status

In PPMI 2.0, two variables were added that capture information a new exam category: Mental Status. These variables capture both the overall exam result (MNTLRSP) and any comments (MNTLCM). This was not captured in PPMI 1.0

Motor

In PPMI 1.0, the motor category was named “Motor System”. It had two sub-categories: Muscle Strength and Coordination (see above). The Muscle Strength sub-category had eight variables, four of which contained the exam result for the specified body region (MSLARSP, MSLLRSP, MSRARSP, MSRLRSP), and four variables that contained any comments for that region (MSLACM, MSLLCM, MSRACM, MSRLCM).

In PPMI 2.0, the motor category is named “Motor Exam”. It contains two variables that capture both the overall exam result (MTRRSP) and any comments (MTRCM).

Notably, what was captured in the PPMI 1.0 “Muscle Strength” sub-category does not encompass all that is captured in the Motor Exam category in PPMI 2.0.

Reflexes

In PPMI 1.0, the Reflexes exam category had two sub-categories: Muscle Stretch Reflexes and Plantar Response.

Within the Muscle Stretch Reflexes sub-category, there were 8 variables, four of which contained the exam result for the specified body region (RFLLARSP, RFLRARSP, RFLRLRSP, RFLLLRSP), and four variables that contained any comments for that region (RFLLACM, RFLLLCM, RFLRACM, RFLRLCM).

Within the Plantar Response sub-category, there were 4 variables, two of which contained the result for the specified body region (PLRLRSP, PLRRRSP), and two variables that contained any comments for that particular exam (PLRLCM, PLRRCM).

In PPMI 2.0, the Reflexes section is captured by two variables, one of which contains the overall exam result (RFLXRSP) and one that contains any comments (RFLXCM).

Sensory

In PPMI 1.0, the Sensory section had a Sensation sub-category. This Sensation sub-category included 8 variables, four of which contained the exam result for the specified body region (SENLARSP, SENLLRSP, SENRARSP, SENRLRSP) and four variables that contained any comments for that region (SENLACM, SENLLCM, SENRACM, SENRLCM).

In PPMI 2.0, the Sensory section has only two associated variables, one of which captures the overall exam result (SENRSP) and one that captures any comments (SENCM).

Future State

Some of the conceptual overlap described above between PPMI 1.0 and 2.0 variables can be used to derive the more general 2.0 variables from the more granular 1.0 variables. The extent to which these variables can be derived for PPMI 1.0 data will be determined for a future state.
